I dealt with back pain for over 10 years, and nights were the worst part of it.
You’ve probably already tried PT. Maybe chiropractic care. Maybe injections. You’ve bought the pillows and swapped the mattress.
And you still lie there doing the math on how long you can stay in one position before you have to move.
In this video I’ll show you the 3 sleeping positions my clients use with L5-S1 herniated disc pain, plus the 2-minute move I do before bed that changed my nights.
First, why nights are harder. You’re still for hours, your tissues stiffen a little, and when you’re tired and stressed your body’s alarm system is more sensitive. The pain at night is real, but it usually isn’t new damage.
Then the positions:
Side lying with a pillow between your legs. Bend your hips and knees slightly and keep your shoulders and hips stacked. Don’t force yourself into a tight fetal curl. Try either side and use whichever feels better.
On your back with a pillow under your knees. Use one or two pillows so your knees rest comfortably bent. This can help your lower back muscles let go.
On your stomach with a thin pillow under your hips and lower belly. This is an option if stomach lying already feels relieving, and the pillow reduces excessive arching. Stomach sleeping aggravates some backs, so skip it if your symptoms increase. These setups follow Mayo Clinic’s general back-pain sleep guidance.
Here’s the part most people miss: the position matters less than how tense you are when you get into it.
Bracing all night keeps the alarm on. So I’ll walk you through 5 slow breaths with a long exhale, letting your belly and low back soften.
Then the before-bed move. Lying on your back with your knees bent, gentle knee rocks side to side for 60 seconds, then 5 breaths. Small and easy, no stretching into pain.
And I’ll show you how to get out of bed in the morning without that first-move catch: roll to your side, push up with your arms, one smooth motion.

If nights have been this bad for a while, the positions help. But the bigger fix is calming the system that keeps turning the pain up.
